Temporary Guest Account Request

Professional Education and Lifelong Learning program managers and coordinators may use this form to request UR network guest accounts for individual classes.

Requests must be submitted at least one week prior to the course start date. Required fields are noted with an asterisk (*).


First and last names of guests. Multiple names separated by a comma, e.g. John Doe, Jane Doe, Sarah Jones, Edward Smith.
